The multi-disciplinary co-located Transitions Team work with disabled young people and those with additional needs, 14-25, and support them through changes they and their families face as they grow up. For example planning opportunities for education, living independently, controlling any health issues, travelling on public transport, moving into work and positive activities in the community.
The team, made up from four groups of professionals, Connexions, advice and guidance; YILTS, independent travel training; social workers on the Children’s Health and Disability Team and Adult Transitions Care Mangers and Transitions Co-ordinator are now based together in the Transition Zone at Applefields School to offer these young people a coordinated support service as they move into adulthood.
