The Avenues Health Visiting Team

Heworth, York

Description

Health visitors support all pre school children and their families, (from pregnancy to starting school) by implementing and supporting the national Healthy Child Programme and the Public Health agenda.
The health visiting teams are based in CYC children’s
centres or NHS health clinics and they include Health
Visitors, and Child Development Workers (nursery nurses) and can also include staff nurses.
The Health Visiting Team promotes or enhances children and family's health and wellbeing. They work in partnership with families and communities and their local Children Centres and liaise closely with GPs, Midwives, School Health, other health and social care professionals, and the voluntary sector. Health visitors provide proactive and preventative strategies and care plans to promote safeguarding, protection of children and vulnerable adults, and healthy lifestyles.
The teams provide sensitive and effective parenting education, confidence and skills to encourage readiness for school.
They may see you at home, in health settings, and children’s centres. You can talk to your health visitor team about any concern you may have about you, your pre-school child and the family.
